Major shift in understanding how eczema develops

In patients with eczema, or atopic dermatitis, the most common inflammatory human skin disease, the skin barrier is leaky, allowing intruders -- pollen, mold, pet dander, dust mites and others -- to be sensed by the skin and subsequently wreak havoc on the immune system. While the upper-most layer of the skin -- the stratum corneum -- has been pinned as the culprit in previous research, a new study published in the Journal of Allergy and Clinical Immunology found that a second skin barrier structure, consisting of cell-to-cell connections known as tight junctions, is also faulty in eczema patients and likely plays a role in the development of the disease.

Currently, there are no treatments that target skin barrier dysfunction in eczema. Pompholyx (dyshidrotic eczema, vesicular hand eczema). DermNet NZ. Facts about the skin from DermNet New Zealand Trust.

Pompholyx is also known as dyshidrotic eczema or vesicular eczema of the hands and/or feet. The term 'dyshidrotic' implies there is something wrong with the sweat duct, but this is incorrect despite the condition sometimes being aggravated by heat and stress, which could increase sweating of the palms and soles. Clinical features of pompholyx The first (acute) stage of pompholyx shows tiny blisters (vesicles) deep in the skin of the palms, fingers, instep or toes.

The later and more chronic stage of pompholyx shows more peeling, cracking, or crusting. Severe pompholyx around the nail folds may cause nail dystrophy, resulting in irregular ridges and chronic paronychia (nail fold swelling). More images of pompholyx ... Complications of pompholyx.
