Transition Assessment. The regulations for the Individuals with Disabilities Education Act of 2004 require "appropriate measurable postsecondary goals based upon age appropriate transition assessments related to training, education, employment, and, where appropriate, independent living skills" (§300.320(b)(1)).

Transition assessment is not defined in the law or the regulations, but the following definition is widely used: "Transition assessment is an on-going process of collecting information on the student's strengths, needs, preferences, and interests as they relate to the demands of current and future living, learning, and working environments. This process should begin in middle school and continue until the student graduates or exits high school.
